We will be providing you with a copy of our Franchise Disclosure Document. By filling out the information below you acknowledge and agree that you were given the following notice and selected the format you prefer prior to being provided with a copy of the Franchise Disclosure Document:

If you qualify as a franchisee candidate, or if you are an existing franchisee requesting additional territory or certain other changes to your franchise, we will provide you with our Franchise Disclosure Document ("FDD"). We may send you our FDD in electronic form as a pdf file on a CD-ROM ("CD") or through email linking you to a secured website. You may review and print our FDD from the CD or secured website. You will need to have Adobe Reader® software installed on your computer to view the document and a printer to create a paper copy of the document. You will also need a computer that reads CDs. If you prefer, we may furnish you with our franchise disclosure document in paper form.

If you visit our office we will deliver the document to you personally. If you request that a copy be sent to you, we will mail or courier it to you in the format you have selected at our cost.
